• The FLEOA 111 Project, via our payment processor Mad Rooster Industries (UEI: DUUDD8JNK1F9), will accept the PO and reserve the spot(s) upon your confirmation of these terms. For multiple registrations from a single agency, we can accommodate a single PO for multiple attendees or one per attendee, whichever is preferable..

• Following successful completion of the training, our payment processor, Mad Rooster Industries, will issue an invoice for the full amount ($100 per participant) referencing the PO number.

• Payment will be due within 15 calendar days of the invoice date.

Instructions for Registration with Purchase Order:

1.  Send the Purchase Order to 111project@fleoa.org

 The purchase order must include:

• PO number

• Total amount, $100/per registrant

• Registered participant name(s)

• Billing contact information - name, email, direct phone number

2.  Register below. In the “Order Number” field, enter the PO number in this format “PO####.”
